H1N1 Vaccination Clinic Information

Dear Parent/Guardian:
The Onondaga County Health Department will be offering free H1N1 vaccinations to all PreK-12 children enrolled in school districts in Onondaga County. As a result, in order to better meet the needs of all Onondaga County students, some school districts will share a clinic location. This will be the case for our Onondaga Central School District.
The Onondaga Central School District is part of a combined effort with the Fabius-Pompey Central School District and the LaFayette Central School District. The combined clinic will be held on November 23, 2009 in the Onondaga Jr/Sr High School Gymnasium, 4479 South Onondaga Road, Nedrow, between the hours of 4:00-9:00 p.m.
If you wish to have your school-age child receive the H1N1 vaccine, please register in advance. You can register through the Onondaga Central School District website (www.ocs.cnyric.org), or by calling the Onondaga County Health Department flu hotline at 435-5752 between the hours of 9:00-4:00 p.m. The best way for the Onondaga County Health Department to provide an efficiently run clinic is to have a good estimate of the number of children attending the clinic.

JUST THE FACTS
1. The combined clinic for Onondaga Central School District, Fabius-Pompey School District, and LaFayette School District students will be held: Date: November 23, 2009 from 4:00-9:00 p.m.
Location: Onondaga Jr/Sr High School Gymnasium 4479 South Onondaga Road (Rte. 80) Nedrow, New York 13120
Please note: If the November 23rd clinic is cancelled due to a lack of vaccine, it will be rescheduled on December 1, 2009.
2. You may register your child through the Onondaga Central School District website (www.ocs.cnyric.org) by going to the COMMUNITY link and then click on the Health Connections page. At the bottom of the page, click on the link Onondaga County Health Department H1N1 Vaccination Clinic Registration to complete the necessary registration process.

You may register by calling the Onondaga County Health Department flu hotline 435-5752), 9:00-4:00 p.m.
3. Please plan to accompany your child to the clinic. A signed parental consent form is required prior to receiving the vaccine. Consent forms will be available at the vaccination site for those parents who will be accompanying their child(ren).
Note: If you plan to have another adult accompany your child to the vaccination clinic, you must obtain a consent form from your child’s school office (prior to November 23rd)and send the consent form, signed by the parent, with the adult who will be accompanying your child(ren).
4. Be prepared to wait - consider bringing books, games, possibly snacks. Please have an alternate care plan in place for younger siblings.
5. DO NOT bring your child to the clinic if he/she has any symptoms of flu-like illness.
